PLEASE READ CAREFULLY

Participation in athletic training, speed training, strength training, plyometrics, sprinting, jumping, conditioning, and competitive sports activities involves inherent risks, including but not limited to strains, ligament injuries, fractures, concussions, heat illness, cardiovascular events, permanent disability, or death.

ASSUMPTION OF RISK

I voluntarily choose to participate, or allow my child or ward to participate, in training, instruction, competitions, camps, virtual programs, and related activities provided by PR1M3 Performance LLC, its owners, coaches, trainers, contractors, and affiliates (“PR1M3 Performance”). I knowingly and freely assume all risks, both known and unknown, associated with participation, even if arising from the ordinary negligence of PR1M3 Performance or others, except as stated below.

RELEASE & WAIVER OF LIABILITY

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless PR1M3 Performance LLC and its owners, employees, coaches, agents, and representatives from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation in training or competition, including injuries or damages caused by ordinary negligence. This waiver does not apply to injuries or damages resulting from gross negligence, reckless conduct, or intentional misconduct.

MEDICAL RESPONSIBILITY

I certify that I or my child/ward is physically able to participate and has no medical condition that would prevent safe participation, or that such conditions have been disclosed. PR1M3 Performance is not responsible for providing medical care. I authorize emergency medical treatment if necessary and accept full responsibility for all related medical expenses.

PARENT / GUARDIAN CONSENT

If the participant is under 18 years of age, I certify that I am the parent or legal guardian and consent to participation under the terms of this waiver.

MEDIA RELEASE

I grant PR1M3 Performance permission to use photos and videos taken during training or events for marketing, educational, and promotional purposes without compensation.

GOVERNING LAW

This agreement shall be governed by and interpreted under the laws of the State of New York.

ACKNOWLEDGMENT

I acknowledge that I have read and fully understand this waiver and that by agreeing, I am giving up certain legal rights, including the right to sue, except in cases of gross negligence or intentional wrongdoing.
